Insurance Plans

Mountain Utah Family Medicine participates with most area insurance plans, and we will be
happy to submit your insurance claims for you. Any questions regarding your specific coverage
and benefits, referrals and authorizations should be directed to your insurance company or
your company benefits administrator. You are responsible for any amount not covered by your
insurance plan. Below is a partial list of insurances we are contracted with. If your insurance is
not listed, you will need to contact your insurance company to verify if we are contracted with
them. If your insurance is listed below, it is still not a guarantee we are contracted and in
network with your particular plan. Ultimately you are responsible to know your benefits and if
your visit with us will be covered or not.
