Do I need insurance to receive medical services?  

We serve all patients, regardless of insurance. Payment is required at the time of service for all non-insured patients. We accept payments in the form of cash, check, or credit cards.

 

Do you accept my insurance?

Our clinic partners with most insurance companies. If you would like to verify your insurance coverage at our clinic, give your insurance company a call for the most up-to-date information.

 

Have you billed my secondary insurance?

Once we have received a response from your primary insurance, we will bill your secondary insurance. Check your most recent statement to see which insurance was most recently billed by our medical clinic.

 

Does Mountain Utah Family Medicine require a co-payment prior to the appointment?

Without a co-payment ready at the time of your appointment, you may be asked to reschedule. A co-payment is a prerequisite of your insurance company.

 

How can I pay my Mountain Utah Family Medicine bill?

  • In-Person: Drop off your payment to our office anytime during office hours.
  • Telephone: We can process your payment via telephone by contacting us at (435) 893-2959.
  • Mail: In addition to your statement, send your payment to our address listed below.

Why does my statement not correctly reflect my past payment?

It may take up to 10 days for your payment to be posted to your account. Please contact our clinic  at (435) 896-9561, if it has been more longer than 10 days. We will be able to assist you.

 

When is a referral required?

A referral may be necessary in unique circumstances when you need to schedule an appointment with a doctor other than your primary care physician. Please read your complete insurance policy to ensure the referral is covered by your insurance.
